| Issued date | Select the document issued date from the date picker. | Required | Dropdown |
| Expiry date | Select the document expiry date from date picker so that system will send you reminder notifications and emails to renewal them. | Optional | Dropdown |
Click on Add File to upload document. After uploading the document click on submit button, that document can be seen on right side of the screen.
